All student forms must be lodged online through the iStudi Student Portal.
Paper or email submissions will not be accepted.
How to Access the Forms
- Log in to iStudi (Student Portal) using your student ID and password.
- Go to Requests → Student Forms.
- Select the form you need from the list above.
- Complete all required fields and upload supporting documents (if applicable).
- Submit the form. You will receive a confirmation email and a ticket number.
- Track your submission under My Requests in iStudi.
